Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Massachusetts Physical Therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Minimum 1 year of certified home health experience within a certified home health agency (required).
• Minimum 1 year of case management experience.
• Ability to travel to patient cases as assigned.

Assignment Details

• Facility Type: Home Health.
• Shift Name: Days.
• Shift Details: Days 0800-1630.
• Weekend rotations: once every 6 weeks.
• Start: 05/31/2026. Assignment length: 13 week.
• Candidates must perform admissions, case management duties, and timely electronic documentation in accordance with agency policies.
