Admissions Coordinator for SNF
Job Summary:
Civita Care Center at Cheshire is seeking a highly skilled and compassionate Admissions Coordinator for our skilled nursing facility (SNF) in Cheshire, Connecticut.
Responsibilities:
Coordinate and facilitate the admission process for patients transferring from hospitals to our SNF, ensuring seamless communication with healthcare providers and families
Conduct thorough assessments of patients' needs, including medical history, treatment plans, and social services requirements
Develop and maintain relationships with local hospitals, physicians, and other healthcare partners to promote our services and build strong referral networks
Collaborate with our interdisciplinary care team to create personalized care plans that meet each patient's unique needs and goals
Monitor and report on patient outcomes, identifying areas for improvement and implementing process changes as needed
Participate in performance improvement initiatives and quality assessment activities
Maintain accurate and detailed records, adhering to regulatory standards and facility policies
Requirements:
Bachelor's degree in a related field (e.g., healthcare administration, nursing, social work)
Minimum of 2 years of experience in healthcare administration, admissions, or a related field
Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals
Ability to work in a fast-paced environment, prioritizing tasks and managing competing demands
Familiarity with medical terminology, insurance policies, and healthcare regulations
