They are responsible for outreaching and scheduling member face-to-face visits in homes, dialysis clinics, and or physician offices to support higher risk members needing barrier assessments and face-to-face care plan education. In addition, our field nurses develop trusting relationships with the nephrology practices in their markets and work closely with those practices to support improvement in members health outcomes.This is a field-based position meeting with members in their homes and facilities and will be in the field at least 75% of the time. The territory will primarily cover the greater St. Louis area.Provide a complete continuum of quality care through close communication with members via in-person or on-phone interactionTravel to complete in home and in facility visits with membersTravel to participate in care planning in facilities & provider officesUse nursing assessment skills to identify medical, behavioral, social determinates of health gaps or barriers in treatment planProvide condition education, conduct medication reviews, assist with advance planning and connect to community resourcesDemonstrate ability to establish trusting relationships with members & caregivers and identify innovative strategies to improve and maintain engagementParticipate in interactions with interdisciplinary team, treating providers and community resources.Complete in-patient & post-discharge visits to ensure that discharged members receive the necessary services and resources to focus on reducing readmissionsDocumenting and track findings; solid computer and software navigation skills are criticalComplete and maintain field based safety trainingsQualifications:Ability to get licensed in other states as needed3+ years of RN experienceRenal, Chronic Kidney Disease or Dialysis Care experience as a main focus of your jobComputer proficiency, to include strong data entry, utilizing MS Office (Word, Excel, PowerPoint and Outlook), and the ability to type and talk at the same time while navigating a Windows environmentReside in a location that can receive a high speed internet connection or can leverage existing high-speed internet servicePreferred Qualifications:BSN or higher level of educationCertified Case Manager (CCM)Diabetic Educator experienceField-based experience going into homesTelephonic case management experienceExperience working within a managed care environmentThis job description is not designed to cover or contain a comprehensive listing of activities, duties, or responsibilities required of the employee.
