Long-Term Care LPN Job Details

• Position: Licensed Practical Nurse (LPN)

• Orientation: Full-day orientation on a Tuesday or Thursday, including two 12-hour orientation shifts
• Contract Duration: 13 weeks
• Start Date: November 19, 2026
• End Date: February 20, 2027
• Weekly Estimated Pay: $1,400–$1,470

LPN Job Requirements

• At least 1 year of experience working as a Licensed Practical Nurse in a long-term care or skilled nursing facility
• Current New York LPN license
• Current Basic Life Support (BLS) certification through the American Heart Association or RQI
• Ability to provide safe, appropriate nursing care to adult and geriatric residents
• Knowledge of age-specific assessment, growth and development principles, and individualized resident needs
• Strong communication and collaboration skills for working with residents, families, nurses, providers, and interdisciplinary team members
• Ability to comply with infection prevention, workplace safety, and bloodborne pathogen precautions

Long-Term Care LPN Responsibilities

• Provide direct and indirect skilled nursing care to assigned long-term care residents under the supervision of a Registered Nurse.
• Assess and collect data related to residents' physical, psychosocial, and age-specific needs.
• Communicate significant findings and changes in resident condition to the Registered Nurse and interdisciplinary care team.
• Contribute to individualized, developmentally appropriate, and culturally responsive plans of care.
• Implement nursing interventions designed to support resident safety, comfort, health, and desired outcomes.
• Evaluate resident responses to care and recommend updates to the plan of care within the scope of LPN practice.
• Identify the need for referrals and collaborate with residents, families, and healthcare team members to address comprehensive care needs.
• Maintain current clinical knowledge and competency through continuing education and in-service programs.
• Promote a safe, respectful, and supportive environment for residents, families, and fellow team members.
