Licensed Practical Nurse Job Requirements

• Active New York LPN license
• Current Basic Life Support (BLS) certification through the American Heart Association or RQI
• At least 1 year of experience working as an LPN in a long-term care setting
• Ability to provide safe, compassionate care to adult and geriatric residents
• Strong communication and collaboration skills for working with residents, families, nurses, providers, and interdisciplinary team members
• Ability to work scheduled day or night shifts, including every other weekend and holidays

Long-Term Care LPN Responsibilities

• Provide direct and indirect skilled nursing care to assigned long-term care residents under the direction of a Registered Nurse (RN).
• Collect and document data related to residents’ physical and psychosocial conditions, and promptly communicate findings to the RN and interdisciplinary care team.
• Contribute to individualized, age-appropriate, and culturally responsive plans of care.
• Implement nursing interventions, evaluate resident outcomes, and recommend updates to the plan of care within the scope of practice.
• Support a safe, healthy, and comfortable environment for residents.
• Collaborate with residents, families, nursing staff, physicians, and ancillary departments to promote continuity of care and resident satisfaction.
• Maintain current nursing knowledge and clinical competency through continuing education and in-service programs.
• Participate in staff mentoring, orientation, and professional practice initiatives as appropriate.
• Follow established policies and procedures while maintaining awareness of safety considerations and occupational exposure risks.
