Home Health Registered Nurse Location: Palatine, IL, USA (additional locations include Huntley, Crystal Lake, and surrounding areas). Salary: $10,000 first‑year success bonus available. Responsibilities Lead and collaborate as case manager to develop and implement patient care plans. Provide direct nursing care to patients in the home setting. Document care and charting in a timely manner. Coordinate with interdisciplinary team and support flexible scheduling. Ensure safe and accurate medication administration and supply management. Qualifications RN license in Illinois (or the state of practice). Graduate of an approved practical nursing school. Minimum one year of RN experience in a clinical setting. Current driver’s license and ability to travel approximately 20% of work time. Strong time‑management and self‑management skills. Proficiency with tablets, mobile phones and EMR software.
