Helms Home Care is seeking a dedicated After Hours Patient Care Coordinator to join our growing team. This role is vital in supporting patients receiving home infusion therapy and ensuring smooth coordination between pharmacy vendors, nursing staff, and patients.Primary Responsibilities:Accept and process new patient referrals from various vendorsSchedule home infusion visits and maintain accurate records in the scheduling systemCommunicate with pharmacy vendors and nursing staff regarding patient care, supply needs, and scheduling updatesSupport overall patient care coordination with professionalism and attention to detailWork Schedule:Tuesday: 4:00 PM – 8:00 PMWednesday: 4:00 PM – 8:00 PMThursday: 4:00 PM – 8:00 PMFriday: 4:00 PM – 8:00 PMSaturday: 8:00 AM – 8:00 PMSunday: 8:00am-12:00 PM #J-18808-Ljbffr Helms Home Care Llc
