Mobile RN Care Manager for Dual-Eligible Members

Commonwealth Care Alliance is seeking a Clinical Care Manager for Massachusetts to monitor and coordinate care for dually-eligible enrollees with complex needs. This mobile role involves extensive home visits and collaborations with healthcare providers to improve health outcomes. The position requires an RN license with advanced education (MSW or LMHC preferred) and 3 years of RN/BH clinician experience or 1 year with two years of related experience.
