Job Title: Registered Nurse (RN) - Home Health Profession: Nursing Specialty: Home Health RN Duration: 13 weeks Shift: Flexible Hours per Shift: 40 Experience: Minimum Years of Experience: 0 License: Active Registered Nurse (RN) License in State of Colorado (or active NLC compact license) Certifications: Valid driver's license, reliable transportation, and auto insurance Must-Have: Strong assessment and critical thinking skills Ability to work independently and manage a structured caseload Clear, professional communication with patients and team members Comfort with documentation and EMR systems (HCHB preferred) Home health experience preferred, but not required Description: We are seeking a Registered Nurse (RN) to provide skilled, patient-centered care in the community. This is an in-person home health position, offering flexibility in how you structure your day. The position includes manageable caseloads and meaningful one-on-one care. What You'll Do: Perform comprehensive patient assessments. Develop and update individualized plans of care. Provide skilled nursing care and appropriate interventions. Monitor patient progress and report changes to the care team. Educate patients and families on medications, treatments, and care plans. Coordinate care with LPNs, therapists, caregivers, and providers. Complete timely and accurate documentation in EMR. Participate in on-call rotation.
