UnityPoint Health - Proctor

UnityPoint Health seeks an RN Case Manager to deliver hospice care in patients' homes, applying clinical nursing knowledge, assessment skills, teaching, and care planning under physician direction. You will coordinate interdisciplinary care, perform initial and ongoing assessments, ensure adherence to the hospice plan, and support families throughout the care journey.
