Home-Based Hospice RN Case Manager

West Des Moines, IA

Care Initiatives Hospice is seeking a compassionate Registered Nurse (RN) to join as an RN Case Manager. You will meet patients and families in their home or chosen setting to deliver individualized hospice care with dignity and respect. Responsibilities include comprehensive assessments, developing and overseeing plans of care with the physician, documenting care, and counseling families. Travel within your region and ability to obtain CHPN within 24 months are preferred. #J-18808-Ljbffr
