Job Description:

Responsibilities:

• Coordinate and oversee patient care plans to ensure optimal health outcomes and efficient use of healthcare resources.
• Assess patients' medical conditions, social determinants of health, and care needs to develop individualized care plans.
• Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure continuity of care.
• Coordinate insurance authorizations, appeals, and approvals for necessary medical services and treatments.

Qualifications:

• WA Registered Nurse license
• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
• Basic Life Support (BLS) certification
• Case Management Certification (CCM) or Accredited Case Manager (ACM) certification preferred

Job details:

• Pay: $1,693.09 - $1,745.45 / week
• Location: Tacoma, WA
• Assignment Length: 12
• Shift: Day - up to 36.00 hours per week
• Setting: Skilled Nursing Facility - SNF
• Workplace: On-site
