Case Manager — Empower Education & Community Support

Eoc Nassau

Eoc Nassau is seeking a Case Manager to provide comprehensive case management services, encouraging clients to pursue their education and eventually secure employment. The ideal candidate will have a Bachelor’s degree in Social Work or a related field and experience with high-risk populations. Responsibilities include weekly meetings with clients, connecting them to community resources, and documenting progress.
