RN Case Manager

As a Registered Nurse (RN) on our case‑management team, you will help make communities healthier through compassionate care. More About Our Team The Case Management team serves the adult population, maintaining a patient‑to‑staff ratio below the national average. We coordinate safe, effective discharge planning through close collaboration between a Utilization Review RN, Case Manager RN, and Social Worker. How You’ll Contribute Complete documentation per workflow timeline and content requirements, including the Individual Plan of Care (IPoC) per CMS guidelines. Schedule family conferences and communicate with caregivers following each team conference and as needed to keep them informed of progress and discharge plans. Coordinate weekly patient care team conferences to develop, monitor, and refine treatment plans toward identified goals. Review assigned CMGs, identify potential comorbid conditions in treatment, and communicate findings to the HIM team. Assist with concurrent and retrospective utilization review activities, including denials and appeals, and work with physicians on peer reviews with payer medical directors. Ensure clinical updates are provided to all insurance payers and document payer communications in Meditech. Coordinate discharge planning, including home health, follow‑up care, durable medical equipment, and other services, ensuring a safe thorough discharge plan and documenting patient choice as per CMS conditions. Identify quality and cost‑effectiveness trends and present findings to departmental leadership. Perform intake assessment within 24–72 hours of admission, preferably within 48 hours. Responsibilities include comprehensive assessment, care planning, plan implementation, and overall patient evaluation, serving as a patient advocate and addressing holistic needs. Applicants must have a current VA RN license or compact state license. Additional Requirements Include Bachelor’s degree preferred but not required. Experience in an acute‑care hospital preferred.
