Inpatient Social Worker Case Manager, Care Coordination (Full Time)

$27.31 per hour

Responsibilities Coordinate and implement case management activities encompassing all care needs during the patient’s hospital stay. Serve as the point of contact for the patient, family, or legal representative until the patient transitions to another service line, OP case manager, or patient navigator. Review care and treatment for appropriateness against screening and reimbursement criteria for referral management. Conduct continual monitoring and assessment of the patient’s care plan goals and needs, modifying referrals and resource requests as necessary. Send any ordered or necessary referrals to the appropriate service line or community resource for review of appropriateness of services or resources requested. Attend daily care rounds if applicable, communicating any changes in the patient’s clinical condition that may impact the transitional care plan to the multidisciplinary care team. Communicate and collaborate with multidisciplinary care team members, patient navigators, OP case managers, patients/families/legal representatives. Demonstrate the ability to handle pressure to meet deadlines, to be accurate, and to manage constantly changing situations. Demonstrate the ability to work with a variety of people, handle stressful situations, and resolve conflict. Perform all job duties or assigned tasks as directed. Follow and adhere to all licensing board or agency requirements, regulations, and procedures. Comply with all Benefis Health System organization policies and procedures. Qualifications & Experience BSW preferred. Minimum 2 years of medical/hospital experience. At least 2 years of case management experience preferred. Ability to meet all organizational and regulatory requirements. Compensation Exemption Wage Non‑Exempt Starting Wage $27.31 hourly.
