Pay per Visit Sup/Post Hospital: $60 Reassessment: $75 Initial Admission: $90 weekday/ $125 weekend Visiting Client Territory: Liverpool, and surrounding areas. Participates in the training of new nurses and in‑services for field staff. Performs initial nursing assessments Counsels patient/caregiver in meeting nursing and related needs. Assigns and coordinates services provided. Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient’s condition/needs. Prepares clinical progress notes ensuring any changes in the patient’s condition is reported Monitors clinical records for complete and accurate documentation.
