Responsibilities:
Provide direct patient care to residents, including administering medications, performing vital signs, and assisting with daily living activities
Develop and implement individualized care plans in collaboration with the healthcare team
Conduct regular assessments to identify changes in residents' conditions and report findings to the RN or physician
Maintain accurate and timely records of patient care and treatment
Participate in in-service training and educational programs to stay current with industry developments and best practices
Collaborate with the interdisciplinary team to ensure seamless care delivery and resident satisfaction
