Care Promoter, Specialist Cardiology

Care Promoter, Specialist Cardiology Responsibilities Measure and record patient vital signs; record patient interview and medical history. Collaborate with the care team to ensure specialistrelated medical records and consult notes are obtained. Prepare patients for examinations and perform routine screening tests, assist the physician with exams, and explain treatment procedures and physicians' instructions to patients. Escort patients to exam rooms and ensure their comfort and well‑being before leaving. Observe patients and report any changes in conditions to the specialist, PCP, clinician, or other clinical personnel. Perform non‑critical medical and therapeutic procedures based on medical instructions. Collect phlebotomy and other laboratory specimens; perform basic waived laboratory tests. Prepare and administer medications, change dressings, apply bandages, remove sutures, and perform first‑aid procedures; use CPR skills when necessary under physician supervision. Upon physician approval and authorization, call in prescriptions to the pharmacy. Use appropriate medical terminology and adhere to legal and ethical professional conduct; authorized individuals will use the ChenMed Rx system to support physician medication dispensing. Provide health coaching to a defined group of patients, support healthy lifestyle choices, and follow up with coached patients via weekly calls. Catalog and communicate patient needs to the specialist, PCP, clinician, and other clinical personnel. Maintain supplies, equipment, stocks and sterilize instruments; practice OSHA safety standards and perform accurate, legal, and ethical documentation at all times. Partner with the care facilitator team, greet patients and schedule appointments upon check‑out. Serve as the gatekeeper for specialist schedules, ensuring all patients have follow‑up appointments with PCP and specialist. Work with Care Facilitators to confirm and reschedule upcoming appointments as needed. Work with Care Coordinators to ensure specialist orders are completed and patients are scheduled accordingly. Present patients with a customer‑service survey during check‑out and escalates if needed for immediate service recovery. Process patient phone messages, return calls, and route them to appropriate team members; call patients to obtain and relay pertinent information for the physician. Perform other duties as assigned and modified at manager’s discretion. Qualifications, Knowledge, Skills and Abilities Strong knowledge of medical assistant protocols, terminology, services, standards, policies and procedures. Excellent interpersonal and customer‑service skills with compassion and empathy toward patients and families. Exceptional oral and written communication, time‑management and organizational skills. Ability to collaborate and partner with physicians, clinicians and other team members. Ability to manage complex patient requests from providers. Ability to make independent workload decisions. Problem‑solving mindset focused on resolving patient issues and achieving team goals. Basic phone and computer operation skills. Detail‑oriented to ensure accuracy of reports and data. Ability to maintain effective and organized systems for timely patient flow. Ability to act calmly in busy or stressful situations. Proficient in Microsoft Office Suite (Word, PowerPoint, Outlook, Excel) and related software. Ability and willingness to travel locally or regionally up to 95% of the time to cover other centers. Fluency in spoken and written English. Flexibility to work evenings, weekends, and holidays as needed. Education and Experience Criteria High school diploma or equivalent (GED) required. Graduation from a nationally accredited program in Medical Assistant, Patient Care Technician, EMT, or Paramedic required. Minimum of one year of work experience as a Medical Assistant, Patient Care Technician, EMT, or Paramedic required. BLS certification for healthcare providers required. Experience working with geriatric patients is a plus. Experience with electronic medical records (EMR) preferred. Pay Range $20.20 – $28.83 per hour. Final compensation depends on experience, education, geographic location and other factors. The position may also be eligible for bonuses or commissions.
