Description: The Clinical Nurse is a Registered Professional Nurse who provides safe and competent quality care. Care is based on nursing theory and research to a designated group of patients and their significant others. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care. Utilizes the nursing process based on theory, research, evidence-based practice, and approved organizational nursing standards. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizes all aspects of patient care including teaching, coaching, and planning after hospital care. Delegates to others as appropriate. Educates patients and caregivers about their plan of care, transition of care, promotion of health, and prevention of disease. Communicates effectively and professionally with patients, families, and all members of the Interdisciplinary Patient Care Team. Manages assignments within the Care Delivery Model of Modified Primary Nursing. Demonstrates effective allocation of material resources. Role models the tenets of Relationship Centered Care through various facilitation actions. Considers patient values, preferences, cultural diversity, expressed needs, and knowledge in all aspects of care. Establishes and maintains a therapeutic relationship with patients and families. Uses evidence-based practices to increase understanding of patients' perceptions of care. Initiates service recovery for patients and families who have concerns. Demonstrates caring, respect, compassion, empathy, and active listening. Supports staff use of evidence-based practices to enhance patient care experiences. Practices safe and efficient patient-centered care by identifying and escalating barriers. Demonstrates the clinical skills and knowledge necessary for quality and safety in the patient population served. Implements process improvement strategies based on institutional evidence-based practices and procedures. Engages in formal and informal peer and institutional review processes. Collaborates with interdisciplinary teams to create and implement quality improvement projects. Contributes to quality and safety practices and required compliance measures. Implements principles of high reliability to avoid high-risk behaviors and provide a safe environment. Escalates potential safety hazards or gaps in best practices following protocol. Documents nursing practice in compliance with standards. Facilitates patient throughput on a shift basis. Contributes to the environment of care to support colleagues, patients, and their families. Advances clinical competence in nursing practice. Projects a professional image and communicates with styles that demonstrate respect and compassion. Incorporates ethical principles into decision-making for patients and families. Encourages and demonstrates a spirit of lifelong learning and innovation. Acquires knowledge and skills relevant to the role and patient population. Participates in shared decision-making through practice councils and unit initiatives.
