The primary role of the RN/Case Manager is to oversee and coordinate patient care, ensuring that each patient receives the highest quality and most appropriate care possible. This individual will work closely with physicians, patients, and other healthcare providers to develop and implement comprehensive care plans.Responsibilities and Duties:1. Assess and evaluate patient needs and develop individualized care plans2. Coordinate and communicate with physicians and other healthcare providers to ensure seamless patient care3. Monitor and track patient progress and make appropriate adjustments to care plans as needed4. Provide patient education and support to promote self-management and wellness5. Collaborate with interdisciplinary team members to ensure continuity of care6. Advocate for patients and their families to ensure their needs are met7. Document patient assessments, care plans, and progress notes accurately and in a timely manner8. Oversee and supervise other healthcare team members as needed9. Participate in quality improvement initiatives and performance improvement projects10. Stay current on best practices and evidence-based guidelines in nursing and case managementQualifications and Skills:1. Current Registered Nurse license in the state of practice2. Bachelor's degree in Nursing (BSN) preferred3. Certification in Case Management (CCM) preferred4. Minimum of 2 years of experience in nursing, with at least 1 year in case management5. Strong communication and interpersonal skills6. Ability to work independently and as part of a team7. Excellent organizational and time management skills8. Knowledge of healthcare laws and regulations9. Proficiency in electronic health record systems10.
