Transitions Program Lead for Youth & Housing Services
ACH Child and Family Services
The ACH Child and Family Services Transitions Program Manager will lead the LIFE Project and Families Together Program, supervising mentors and the case manager, and overseeing daily operations to ensure high quality services and compliance with grant and contract requirements. You will build community partnerships, monitor program outputs, support staff development, and coordinate referrals with CPS/DFPS as part of delivering trauma-informed, evidence-based services to youth aging out of foster #J-18808-Ljbffr ACH Child and Family Services
