HCR Home Care

HCR Home Care in New York seeks an RN Case Manager to coordinate skilled nursing services, manage a full caseload, and ensure care plans align with physician orders. You will supervise and teach family members and other nursing staff, conduct admissions, evaluate patient needs, and maintain compliant documentation.
