Per Diem Registered NurseNorth Shore Community Health (NSCH) is an industry leader in exceptional community healthcare delivery. Under the general direction of the Clinical Nurse Manager, the Per Diem Registered Nurse provides direct and indirect nursing care and patient education within the scope of their licensure.This position offers flexible scheduling with a combination of onsite and remote work. Per Diem Registered Nurses are required to work at least one onsite shift per month at one of NSCH's health centers (Salem, Peabody, or Gloucester). Additional shifts may be completed remotely or onsite based on departmental and patient care needs. The nurse should also be willing to assist with or learn triage and Medication for Opioid Use Disorder (MOUD) care via Telehealth to support patient access and continuity of care.The Per Diem Registered Nurse will manage medication refill requests, address patient needs both in person and by telephone, and collaborate with the triage nursing team to assess patients and coordinate appropriate care. The Registered Nurse will work closely with the Clinical Nurse Manager, Medical Director, providers, and interdisciplinary care team to promote a safe, efficient, and patient-centered care environment. We do not and will not make any personnel decisions (like recruiting, hiring, job assignments, and promotions) based on age, color, national origin, citizenship status, physical or mental disability, race, religion, creed, gender, sex, sexual orientation, gender identity, gender expression, genetic information, marital status, status with regard to public assistance, veteran status, or any other characteristic protected by federal, state or local law.Essential Functions:Assesses an individual's basic health status and related patient care needs for patients who call the clinic or who present for scheduled or unscheduled careAnalyzes and interprets data and makes informed judgments related to patient careEvaluates outcomes of basic nursing interventions and initiate changes in care plansCollaborates and communicates with primary care providers and other care team members to ensure quality and continuity of care, including participation in huddles to help ensure a smooth workday and maximize time with patients in the clinic.Reviews refill requests from pharmacy and patients based on medication refill protocol; reconciles patient medication lists and attaches pertinent information necessary for the provider to authorize the refillAdministers immunizations and medications and performs other procedures as needed, including but not limited to EKGs, suture removals, wound dressing changes, ear irrigations, PPD application and interpretation, and blood pressure checksMaintains accurate and timely documentation of all nursing care and patient communication, including phone notes, nursing visit notes, and immunization or medication administration documentationCompletes patient forms to the furthest extent possible when needed within the scope of nursing practice, including prior authorization formsProvides health education to individuals and families to further patient self-management and self-empowerment and to help patients maintain/sustain an optimal level of healthCommunicates with members of the patient's care team outside of the health center as needed, for instance, with specialists or visiting nurses, to help with the coordination of careManages vaccination supply and ordering, including proper vaccination storage and handlingUses information about data trends for self-directed learning and performance improvementProvides coverage at other NSCH clinical sites or call centers as neededPerforming additional duties to fulfill NSCH's mission as assignedRequirements:College Degree from an accredited school of nursing, current license to practice as an RN in the Commonwealth of MassachusettsCurrent CPR certificationAbility to function effectively both as a leader and a member of a team, as well as a proven ability to work independentlyAbility to work in a culturally diverse environmentExcellent communication and interpersonal skillsApproaches work with an eye for problem-solving and with a positive attitudeMaintains a high level of customer service with both patients and coworkersPossesses strong critical thinking skills and can prioritize and address many competing demandsMust recognize and respond quickly to patient and/or unit condition changesPreference will be given to bilingual/bicultural individuals who can speak Spanish and/or PortugueseExperience with and proficiency in the use of electronic health records and strong computer skills requiredSensitivity/skill in working with patients with complex medical, psychosocial, and socioeconomic circumstances, such as patients with significant mental illness, homelessness, poverty, and substance use disordersThe ability to travel to other NSCH sites for meetings and periodic clinical coverage is required.
