Working Hours 30-40 hours per week; 35 hours per week; 20 hours per week; 40 hours per week; 20-30 hours per week. Responsibilities Provide outpatient rehabilitation services in an in‑home setting for older adults. Assess patient needs, develop recovery plans, and deliver physical therapy. Instruction, education, and training of patients and caregivers on skills for independence. Consult with interdisciplinary team to select appropriate therapeutic regimens. Document clinical and billing information. Adapt program and care plan to individual patient needs. Maintain safe environment and enforce elimination of fire and safety hazards. Qualifications Valid Physical Therapy license in the state(s) of practice. Master’s degree in Physical Therapy, or Master’s in related field plus Bachelor’s, or Bachelor’s plus 5 years of progressively responsible PT experience. U.S. equivalent assessment for foreign degrees. Valid driver’s license or reliable transportation to patients’ residences. Strong communication and interpersonal skills.
