The Physical Therapist role includes outpatient rehabilitation services in an in‑home setting for older adults, as well as facility‑based care. The position is available at multiple locations including Vineland, NJ; Cherry Hill, PA; Camden, NJ; and other sites across New Jersey and Pennsylvania. The role may be full‑time or part‑time with flexible hours and offers a healthy work‑life balance. Salary Posted Salary Range: USD $45.00 - USD $52.00 /Hr. Responsibilities Provide one‑on‑one physical therapy to older adults wherever they call home (private residences, senior living communities, independent living, assisted living, memory care). Perform evaluations, develop individualized plans of care, and deliver evidence‑based treatment. Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence. Document effectively within the EMR system in accordance with regulatory standards. Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and fitness specialists. Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place. Provide telehealth/virtual care when appropriate to extend access and flexibility. Assess patient needs, develop recovery plans, and deliver physical therapy. Conduct comprehensive assessment of the entire patient, including history, systems review, and application of appropriate tests and measures. Construct and implement individualized therapy programs designed to restore, reinforce, and enhance physical abilities. Encourage active participation and engagement in therapeutic activities. Maintain therapeutic equipment, order supplies and equipment as necessary. Clean and maintain equipment; identify and report need for repair. Ensure compliance with department and patient treatment schedule, timely clinical and billing documentation. Attend and contribute to patient care, staffing conferences and other related meetings. Provide referrals to other disciplines and agencies for holistic care. Consult with other members of the rehabilitation team to select appropriate therapeutic regimen consistent with patients' needs. Adapt program and/or plan of care according to individual patient needs. Promote a safe environment and enforce elimination of fire and safety hazards. Monitor patient progress, attitudes, and behavior related to rehabilitative potential. Perform routine reexamination as needed/required to modify/progress plan of treatment. Incorporate health and wellness programming into treatment plans for patients. Perform home and community mobility assessments and provide recommendations for safe mobility. Design, fabricate, fit and/or apply adaptive devices including orthotics and prosthetics. Instruct patients on use of adaptive devices. Perform other related duties as required. Qualifications Must have a valid Physical Therapy license in the state(s) of practice, or proof of license and eligibility. Must have a Master’s degree in Physical Therapy; or Must have a Master’s degree in a Physical Therapy related field in addition to a Bachelor’s degree in Physical Therapy; or Must have a Bachelor’s degree in Physical Therapy plus 5 years of progressively responsible PT experience. The qualifying Physical Therapy degree earned outside of the U.S. must be evaluated to be the U.S. equivalent. Must have a valid driver’s license or reliable, independent transportation to and from patients’ residences. Physical Therapist Licensure in good standing in the state of New Jersey. Degree from an accredited Physical Therapy program. Basic computer and EMR proficiency. Current CPR certification. BLS certification (for travel assignments). Doctorate degree in Physical Therapy (preferred for travel/leadership roles). Right‑to‑work in the United States. Location & Hours Vineland, NJ – Full‑time 30‑40 hours/wk, salaried; offering $5,000 bonus. Cherry Hill, PA – Physical Therapist – Camden, NJ; mobile outpatient therapy. Greater Lansdowne, PA – Full‑time/Part‑time – salaried with benefits. Deployable travel assignment – 13‑week contract in Broomall, Pennsylvania.
