Social Services Director Heights Care and Rehab | Full-Time | $70,000–$85,000/year Want a job where you can actually make a difference? This isn’t a position where you’ll spend your entire day buried in paperwork. What You’ll Do As our Social Services Director, you'll play an important role in both the resident experience and our interdisciplinary team. You'll: Build strong relationships with residents and their families Advocate for residents and help ensure their psychosocial needs are met Coordinate discharge planning and help residents transition safely to their next level of care Work closely with Nursing, Therapy, Activities, Admissions, and Administration Participate in care planning and interdisciplinary meetings Help residents and families navigate challenging situations and difficult conversations Coordinate community resources, services, transportation, appointments, and other resident needs Assist with grievances and concerns and help develop meaningful solutions Maintain required documentation and ensure Social Services remains survey‑ready Be an active member of our leadership team and help us continually improve the resident experience Who We’re Looking For We want someone who is organized, approachable, dependable, and great with people. Requirements: At least 1 year of relevant Social Services experience Experience in skilled nursing, long-term care, healthcare, or a related setting strongly preferred Strong communication and problem‑solving skills Ability to communicate professionally with residents, families, staff, and outside providers Comfortable managing multiple priorities in a fast‑paced environment Someone who takes ownership and follows through Most importantly, we're looking for someone who cares about the people behind the paperwork.
