Day Habilitation - Direct Support Professional

Direct Support Professional Location: Day Habilitation (Ocean County) Shift: 8 am – 4 pm (Monday – Friday) The Direct Support Professional (DSP) will provide services to individuals with intellectual and developmental special needs. The DSP is responsible for carrying out prescribed individual service plans that include daily living activities, transportation, ambulatory and non‑ambulatory services, and monitoring/administrating medication. These services may be provided in the community, at facilities, or in homes to support the achievement of service recipients’ goals. The DSP delivers quality services and builds strong relationships with internal and external stakeholders, positioning Easterseals New Jersey as the service provider of choice. This position requires flexibility to work in multiple locations and perform necessary assignments that accommodate the needs of service recipients. Essential Functions Works flexible hours and assignments in multiple locations, including IDD Population Health – Residential and Day Habilitation. Travels throughout designated site locations. Transports service recipients to and from home and assigned community activities. This role requires driving; candidates must have a valid driver’s license and will undergo post‑offer verification of driving eligibility consistent with company insurance requirements. Bends, crouches, lifts, pulls, or pushes heavy objects, including wheelchairs, and provides physical assistance and crisis interventions. Position Duties And Responsibilities Provides services as outlined in the service recipient’s plan of care, including transportation, feeding, bathroom transfers, ambulation assistance, hygiene, and medication monitoring/administration, covering all adult daily living skills. Ensures services comply with all regulatory state/federal requirements and Easterseals policies and procedures. Accurately and timely completes regulatory documentation and service reports to assess quality and utilization outcomes using the Easterseals electronic health record system. Generates and follows daily activity schedules for service recipients to meet Medicaid and other funding requirements. Adheres to fleet safety policies regarding driving safety and proper care and maintenance of vehicles. Cares for a clean and safe environment for service recipients and staff, ensuring proper care of equipment, supplies, and vehicles; immediately reports safety concerns to the site supervisor. Reports incidents of abuse, neglect, and exploitation, and protects confidentiality and HIPAA laws. Applies behavior modification plans and techniques as required. Delivers services in compliance with all licensing, regulatory, accreditation, and Easterseals New Jersey policies and procedures, and cooperates with inspections and investigations. Builds and maintains positive relationships with service recipients, care providers, funders, coworkers, and the general public. Maintains compliance with all training, certifications, and credentials required by regulation. Attends staff meetings to stay informed of updated regulations. Cooperates in inspections and investigations. Qualifications And Experience High School diploma or equivalent required. Current driver’s license required – must be in good standing according to DDD standards and Easterseals Fleet Safety Policies. Preferred experience demonstrating competency in supporting and communicating with individuals with intellectual and developmental disabilities. All candidates must undergo criminal background checks, Central Registry, Child Abuse Registry, and drug testing. Effective written and verbal communication. Planning and organizational skills necessary to manage multiple priorities in a fast‑paced environment. Works well with stakeholders and teams. Exercises good judgment and makes decisions. Computer proficiency and ability to learn new applications quickly. Ability to drive large vehicles such as Busettes and 12‑passenger vans (ambulatory and non‑ambulatory).
