Work Environment\n

This role will operate within a virtual setting while coordinating efforts with a variety of stakeholders in our hospice center relative to children’s care. Our remote work environment is designed to ensure that our team members receive the necessary tools and support to thrive, providing flexibility while maintaining high-quality care and service delivery.

Key Responsibilities\n

As a Licensed Clinical Social Worker, you will:

• Conduct comprehensive evaluations and assessments for infants and toddlers to identify developmental delays and disabilities.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSP), ensuring alignment with each family’s unique needs and goals.
\n
• Monitor and assess outcomes as part of a collaborative team, contributing to the overall effectiveness of our early intervention initiatives.
\n
• Provide targeted support and resources to families, helping them foster their child’s development in meaningful ways.
\n
• Comply with all federal and state regulations, ensuring adherence to the highest standards of care.
\n
• Utilize the Natural Learning Environment Practices to develop joint plans that enhance child learning and family engagement.
\n
• Implement evidence-based strategies to help families navigate the challenges of raising a child with developmental delays.
\n
• Utilize coaching techniques and natural environment teaching as part of the intervention plans.
\n
• Establish rapport and trust with children, families, and team members to cultivate a nurturing learning environment.

Patient Population / Client Focus\n

You will work closely with infants and toddlers facing developmental challenges, as well as their families. Your approach will focus on promoting skills across various developmental domains—physical, cognitive, social/emotional, and communication. By providing individualized support, you will empower families to enhance their children's growth and navigate early intervention services effectively.

Qualifications\n

To be considered for this role, candidates must possess:

• A Master’s Degree or Doctorate in Social Work.

• Licensure by the Department of Public Health with LCSW credentials.
\n
• A current and valid driver’s license.
\n
• Strong interpersonal skills and the ability to adapt communication styles to meet the needs of diverse families.

• A commitment to utilizing Natural Learning Environment Practices in service delivery.

Compensation and Benefits\n
