Position Title: Social Worker – Skilled Nursing Facility Department: Social Services Employment Type: Full-Time Setting: Skilled Nursing Facility / Long-Term Care / Rehabilitation

Position Summary

We are seeking a compassionate, dedicated, and experienced Social Worker to join our team at a Skilled Nursing Facility (SNF). The ideal candidate will have previous experience working in a skilled nursing, long-term care, or rehabilitation setting and possess a strong understanding of resident advocacy, psychosocial assessments, discharge planning, and care coordination.

The Social Worker will provide emotional support, counseling, and social services to residents and their families while collaborating with interdisciplinary healthcare teams to ensure high-quality, resident-centered care.

Key Responsibilities

• Conduct comprehensive psychosocial assessments for newly admitted residents and reassess residents as needed.
• Develop, implement, and maintain individualized social service care plans based on residents' needs.
• Provide emotional support, counseling, and crisis intervention to residents and their families.
• Coordinate discharge planning, including arranging home care services, community resources, medical equipment, and appropriate placement.
• Collaborate with nursing staff, physicians, rehabilitation therapists, case managers, and other healthcare professionals to ensure continuity of care.
• Participate in interdisciplinary care plan meetings and resident/family conferences.
• Assist residents and families with adjustment to long-term care placement, illness, disability, and changes in functional status.
• Advocate for residents' rights, dignity, independence, and overall well-being.
• Assist with Medicaid applications, insurance-related concerns, and referrals to community resources as appropriate.
• Address resident grievances and concerns while promoting positive relationships between residents, families, and facility staff.
• Maintain accurate and timely documentation of assessments, care plans, progress notes, and discharge planning activities.
• Ensure compliance with federal and state regulations, CMS guidelines, and facility policies.
• Support facility survey readiness by maintaining appropriate documentation and participating in quality improvement initiatives.
• Provide support with end-of-life care planning, hospice referrals, and advance directives when appropriate.

Required:

• Bachelor's degree in Social Work (BSW) or a related human services field, consistent with applicable state and facility requirements.
• Previous social work or social services experience in a Skilled Nursing Facility (SNF), nursing home, long-term care, or subacute rehabilitation setting.
• Knowledge of psychosocial assessments, care planning, and discharge coordination.
• Familiarity with CMS regulations, resident rights, and long-term care compliance requirements.
• Strong communication, interpersonal, organizational, and problem-solving skills.
• Ability to work collaboratively with multidisciplinary healthcare teams.
• Experience providing support to elderly, disabled, and medically complex populations.

Preferred:

• Master's degree in Social Work (MSW).
• LMSW or LCSW licensure, as applicable.
• Minimum of 2 years of social work experience in a skilled nursing or long-term care facility.
• Experience with Minimum Data Set (MDS) assessments and interdisciplinary care planning.
• Knowledge of Medicaid, Medicare, and community-based support services.
• Experience with electronic medical records (EMR) and documentation systems.
• Familiarity with nursing home surveys, regulatory compliance, and quality assurance processes.

Ideal Candidate

The ideal candidate is an empathetic, organized, and proactive Social Worker who understands the unique challenges of working in a skilled nursing environment. This individual should be comfortable managing multiple resident cases, communicating with families, coordinating discharges, and supporting residents with complex psychosocial needs.

Prior experience in a Skilled Nursing Facility is strongly preferred.
