CARE NAVIGATOR

Responsibilities Spring Mountain Treatment Center is a 110‑bed mental health facility providing acute crisis stabilization services for adults, teens, and children in a secure inpatient setting since 2001. Our individualized treatment includes a structured, therapeutic environment, intensive physician involvement and oversight, and daily group and educational programs. We provide a caring and healthy environment that gives individuals a chance to make positive changes. We are currently seeking a dynamic professional to join our team as a Care Navigator. This role is responsible for meeting with all eligible patients to discuss the benefits of outpatient services as part of their continuum of care treatment. The Care Navigator communicates eligibility to key members of senior leadership and associated departments. They are responsible for routine collection and data entry of outpatient eligibility into Midas for trending report purposes. They should have a thorough understanding of the Step‑Down Playbook, implementing components as needed and workflow process as it pertains to case management and discharge planning. Presentation and customer service skills are key. Job Duties Provide quality case management and psychotherapeutic services to Spring Mountain population. Provide social histories, discharge planning, case management and psycho‑educational groups to age‑specific populations. Provide family meetings as needed. Consult with Social Services, Utilization Management and Outpatient on patient eligibility. Perform other duties as assigned or required by this position.
