EHE Medical Care Manager — HIV Care & Care Coordination
AIDS Healthcare Foundation
AIDS Healthcare Foundation is seeking an EHE Medical Care Manager to support people living with HIV through the Ending the HIV Epidemic program. You will work with the Healthcare Center team to assess needs, develop action plans, arrange referrals, and educate patients to promote adherence and viral suppression. You will coordinate with providers and community resources, verify ACA eligibility, and help address housing, food, and transportation needs to improve access to care, while upholding #J-18808-Ljbffr AIDS Healthcare Foundation
