RN Clinical Care Coordinator

REF:#2cc914f1b7ec5342 Lake Woods Nursing & Rehab Center is seeking a full-time RN Clinical Care Coordinator to oversee and coordinate nursing care in compliance with facility policies and regulations. Key responsibilities include: Conducting interviews and collecting data for Minimum Data Set (MDS) assessments. Making referrals to healthcare professionals and participating in interdisciplinary meetings. Supervising resident care and assisting physicians during rounds. Engaging in the Quality Assurance process. Compensation includes a $12,000 sign-on bonus and hourly pay ranging from $41.95 to $46.45, based on experience.
