Physical Therapist - Full Time

Position Overview Seeking a Physical Therapist (PT) to join a home health care team, enabling patients to recover at home. Responsibilities Include Performing initial and ongoing assessments of mobility, strength, and function. Administering and interpreting tests to set functional goals and outcomes. Providing written evaluations and care plans outlining needs, goals, and progress. Delivering therapy services per physician orders and maintaining the plan of care through documentation and goal adjustments. Collaborating with healthcare teams, educating patients and caregivers on home exercise programs, and discharge planning. Supervising other personnel (PTA, HHA) as applicable and completing assessments like OASIS where permitted. Ensuring documentation compliance and adherence to policies, standards, and regulations. Qualifications Degree in Physical Therapy from an accredited program. Current, unrestricted state license as a Physical Therapist. Valid CPR certification. Preferred Skills Strong organization, communication, and time management skills. Attention to detail and relationship-building abilities. Experience in home health therapy and OASIS documentation.
