Discharge Registered Nurse*

The Job

Arrowhead Regional Medical Center (ARMC) is recruiting for a Discharge Registered Nurse to ensure safe and efficient transitions of care for medically cleared patients from hospital to home. The discharge nurse identifies patients who meet criteria for a safe discharge and facilitates the process and communicates with all departments as needed to maximize efficiency and workflow to accommodate safe patient discharges. Duties include, but are not limited to, the following:

• Prioritize safe patient discharges; Collaborate and communicate discharge care plans with the care teams to develop and implement a smooth patient discharge plan.
• Identify and minimize delays relating to patient discharges; Evaluate patient's readiness for discharge, ensuring they meet necessary criteria for a safe transition to home.
• Educate patients and their caregivers about post-discharge care, medication management, and follow-up appointments.
• Teach, delegate, and direct both professional and non-professional nursing staff and interpret the needs of each patient to all persons participating in patient care.
• Maintain accurate records in the Electronic Health Record (EHR) of discharge plans and patient interactions to ensure continuity of care.

Arrowhead Regional Medical Center is located on a beautiful 70-acre campus in Colton, California (50 miles east of Los Angeles), a state-of-the-art, public/nonprofit, 456-bed, university affiliated teaching hospital licensed by the State of California Department of Public Health and accredited by The Joint Commission. The hospital houses a regional burn center serving four counties, a comprehensive stroke center, level I trauma center and a freestanding in-patient behavioral health center. Additionally, ARMC operates five community-based, primary care clinics and over 40 specialty care outpatient services. ARMC is the primary teaching hospital for the adjacently located California University of Science and Medicine (CUSM), the Inland Empire's newest medical school.
