Key Responsibilities\n

• Conduct comprehensive assessments to understand the psychosocial needs of seniors, evaluating emotional, mental, and social factors that influence their well-being.

• Develop personalized care plans in collaboration with clients and their families that address identified needs, aspirations, and preferences, ensuring an integrated approach to care.

• Provide compassionate counseling and emotional support, particularly during challenging transitions such as health changes, loss of independence, or bereavement.

• Advocate on behalf of clients to secure their access to necessary community resources, ensuring assistance with services such as transportation, healthcare, and social support networks.

Patient Population / Client Focus\n

You will primarily work with an older adult population, typically aged 65 and above, who may face various challenges such as social isolation, mental health concerns, or chronic health issues. Your role is pivotal in helping seniors and their families navigate these difficulties through tailored interventions and access to community services.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
