Alliance Health at Marie Esther - Activities Assistant Reports to : Activity Director Assistance needed for: Sunday, Monday, Friday Job Qualifications: High School Diploma or equivalent necessary Individualized skills in areas such as crafts, music, education or physical activities helpful Friendly, outgoing personality, with a warm, caring and patient attitude. Willingness to work as part of a team. Essential Job Functions: Assists in planning and implementation of organized program for recreation Provides a planned resident specific, goal directed recreation program for assigned residents/patients; facilitates small group activities as assigned by the director Ensure that all charted recreation progress notes are informative and descriptive of the services provided and indicate the residents/patient's response to the service. Assists in developing a written comprehensive plan of care for each resident/patient recreation program that identifies the problems/needs of the resident/patient and the goals to be accomplished for each problem/need. Follow the established plan of care in providing the daily recreation program. Involve the resident/patient and family members in the assessment and care planning process when possible. Transport residents/patients to and from recreation programs.
