Essential Job Functions Deliver and coordinate patient care in compliance with physician orders. Continuously observe and assess patient condition and report changes. Document all activities, assessments, nursing actions, and care coordination timely. Participate in the development, implementation, and update of the nursing care plan. Take appropriate nursing actions based on assessment to achieve expected outcomes. Implement interventions to prevent or reduce risk. Comply with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act. Apply infection control measures consistent with OSHA standards. Educate patients and families on disease processes, self‑care, and prevention. Maintain competency through educational programs, continuing education units, and skill evaluations. Requirements Graduate of an accredited nursing school. Current, unrestricted state license as an LPN/LVN. Current CPR certification. Proficiency in clinical assessments, documentation, and compliance with nursing policies. Additional State‑Specific Requirements South Carolina – One year of pediatrics experience. California – One year of experience under a current nursing license. Louisiana – One year of experience as a licensed nurse. Continuing education as required by state. Preferences Six months of recent experience in a clinical care setting. Home health experience. Other Skills/Abilities Attention to detail. Time management. Effective problem‑solving and conflict resolution. Good organization and communication skills. Physical Requirements Speak, write, read, and understand English. Travel to client sites. Lift up to 50 pounds. Reposition patients and move equipment without assistance. Prolonged walking, standing, bending, kneeling, reaching, twisting. Sit and climb stairs. Visual and hearing acuity. Strong sense of smell and touch. Respond physically and mentally to emergencies. Environment Work in diverse environments that may involve allergens and other conditions.
