As a team member of our Senior Community Care (SCC) team, we work to provide care to patients in nursing homes, senior housing and assisted living settings. Growing together. You will be part of the primary care team collaborating with all interdisciplinary team members and will support multiple providers. The RN is a flexible team member who works alongside nurse practitioners (NPs) and physician assistants (PAs). Standard Hours: Monday-Friday 8:00 a.m. to 5:00 p.m. EST This is a field-based position traveling to facilities throughout Cuyahoga County (i.e., primarily Western Cleveland - Berea, North Olmstead, & Middleburg Heights, OH) & Medina County (i.e., Medina, OH). Primary Responsibilities Collaborating with all members of the interdisciplinary care team around changes in condition and/or care coordination Triage member needs to the appropriate care team member and level of care Assist with transitions in care: outpatient and inpatient Assist in assuring complete and accurate documentation, medication reconciliation, nursing rounds, and risk assessments What Makes an Optum Career Different? Required Qualifications Active unrestricted Registered Nurse license in your state of residence or ability to obtain within 60 days of hire Certified in Basic Life Support Driver's license and access to reliable transportation that will enable you to travel up to 100% to visit clients and/or patient sites within a designated area Preferred Qualifications BSN degree 1+ years of experience working as a RN (long-term care setting preferred) Experience in geriatric medicine, long-term care, senior living or home care setting Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
