Housing Stability Case Manager
Access, Inc.
Access, Inc. in New York is seeking a Case Manager to provide comprehensive case management services for assigned participants, coordinate care with the Senior Case Manager, and assist with housing placement and entitlements. The role entails regular visits, documentation, and collaboration with an interdisciplinary team. Qualified candidates will have a BA/BS in a related field and a minimum of 2 years in mental health/substance abuse work with diverse populations, plus a valid driver’s #J-18808-Ljbffr Access, Inc.
