Cobble Hill Health Center is a not-for-profit long and short-term care facility providing services to the disabled and elderly for over 40 years.

The CNA plays an integral role in helping us achieve our vision: to consistently exceed expectations and set the standard for excellence in residential and community care.

Duties & Responsibilities:

• Assist residents with activities of daily living, including delivering meals, turning and ambulating residents, and ensuring they receive adequate fluids and nourishment throughout the shift.
• Assist residents with personal hygiene and grooming.
• Observe residents and report any health concerns to the supervising nurse.
• Document resident food and fluid intake.
• Assist residents with feeding and nutritional supplements.
• Make residents' beds and keep living areas clean and tidy.
• Transport residents throughout the facility.
• Assist nurses and other staff members as needed.
• Adhere to professional standards and all facility policies and procedures.
• Comply with all federal, state, and local regulations.
• Maintain the confidentiality of all resident and facility information.

Requirements:

• Active New York State Certified Nurse Assistant (CNA) certification.
• High School Diploma or equivalent.
• Strong English reading, writing, and communication skills.
• Prior CNA experience is a plus.
• Skilled nursing facility experience preferred but not required.
• SigmaCare experience is a plus.
