~Join Bridging Life Hospice and Palliative Care as an Advanced Practice Provider! Palliative care is available at any stage of a serious illness, not just at the end of life. It works alongside ongoing medical treatments to manage symptoms, reduce stress and improve quality of life for both patients and their loved ones. In that uncertainty, families deserve long-term and end-of-life care that is steady, compassionate and clear.
At BridgingLife, we focus on comfort and symptom relief while partnering with families to support quality of life at every stage. Care is provided wherever patients feel most comfortable—whether in their own home, a nursing or assisted living facility or one of our inpatient centers
Perform health status data collection in a comprehensive, systematic manner.

~ Collaborate closely with the Medical Director and Associate Medical Director on patient visits and care plans.

~ Collaborate with Hospice and Palliative Care Directors and interdisciplinary team members to ensure operational excellence.

~ Provide physician backups including prescription writing (e.g., comfort care kits).

~ Offer palliative care consults, recommend treatment adjustments, and coordinate care with primary care physicians.

Active Nurse Practitioner License (NP) from the Maryland Board of Nursing (MBON) or State Physician Assistant licensure

~ Prior experience in hospice, palliative care, or related medical fields is highly desirable
