Health Data Coordinator III

Job Location: Remote Work – New York, Albany, New York, United States of America, 12224. Job Type: Full time; Weekly Hours: 40. Compensation Range: $19.08–$25.77 per hour. Responsibilities Conduct primary care population health management and performance improvement of clinical quality metrics (e.g., breast cancer screening, colon cancer screening, diabetes and hypertension management, pediatric well‑child checks, lead screenings, immunizations) through data collection, tracking, analysis, reporting, and outreach. Provide data‑driven support for operational efforts related to payer contracts, provider compensation incentives, and overall population health initiatives. Collaborate with practice teams on quality and process improvement initiatives, compiling and reporting data trends on quality projects focused on moving clinical care measures. Independently analyze clinical metric dashboards, generate and maintain clinical quality metric reports, identify trends or compliance changes, and report data integrity issues. Drive successful patient outcomes in alignment with value‑based contracts by supporting provider performance, maintaining accurate records, performing patient outreach through gap‑closure management, conducting pre‑visit planning, placing orders or referrals, coordinating care, and identifying gaps in preventive, chronic, or follow‑up care using phone, letters, or patient portal. Maintain excellent record‑keeping of clinical quality metrics; request records from specialty providers; ensure data entry standards; manage NCQA renewal recognition; build relationships with staff; promote best‑practice workflows; develop action plans for performance improvement. Minimum Education & Experience Associate's degree or equivalent and 2 years of related experience, including at least 1 year clinical, operational, quality data collection and reporting experience; or an equivalent combination of education and experience. Knowledge, Skills and Abilities Excellent time management, interpersonal, communication, and organizational skills with attention to detail and ability to work independently. Ability to interface effectively with a broad array of individuals including patients, providers, nurses, managers, and other staff. Effective verbal and written communication skills, regardless of work location.
