Field Case Manager - Homeless Outreach
The People Concern
The People Concern is looking for a full-time Case Manager to join our Santa Monica C3 Team. This role is vital in engaging homeless individuals and connecting them to essential housing and supportive services. You'll work closely with clients to develop individualized service plans and assist them to access public benefits and health treatments. Your role will include working as part of a multidisciplinary team and representing the agency in various community partnerships. A Bachelor's degree and relevant experience with homelessness, mental illness, and substance abuse are required. #J-18808-Ljbffr The People Concern
