Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 260 days Shift: 5x3 days Hours per Shift: Not specified Experience: Two (2) years of experience in a home setting within the last fifteen (15) years License: Current RN licensure in Client Certifications: BLS required before start date if not already certified Must-Have: Bilingual Spanish-English proficiency strongly preferred Description: Urgent need for a registered nurse in home health. Candidates must have recent experience conducting comprehensive assessments of patients. Ability to review health status questions and make evaluations and determinations based on criteria for enrollment. Candidates must work comfortably with technology in the workplace. Knowledge of the principles and practices of professional nursing is required. Familiarity with state laws and regulations governing nursing practices is essential. Excellent organizational, interpersonal, written, and verbal communication skills are necessary. Ability to communicate effectively with multi-generational consumers. Demonstrate sensitivity to the concerns of others. Perform well in a fast-paced, deadline-oriented work environment. Ability to manage multiple complex tasks simultaneously. Strong ability to work independently is required. Job Responsibilities: Perform initial in-home evaluations of patients, including health status, strengths, care needs, and preferences. Guide the development of individualized long-term care service plans. Review medical documentation or health referral forms relevant to the case. Enter evaluation data into electronic forms and transmit as required. Document any concerns or conflicting information that arises during evaluations. Emphasize continuity of care to reduce or eliminate fragmentation and gaps in treatment plans. Discuss healthcare options, supports needed, service vendor options, and waiver options with consumers. Initiate communication with consumers for enrollment into managed care health plans. Enhance communication and collaboration with interdisciplinary care team members to improve care coordination. Collect quality review data and documentation for outcome measurements. Record case notes into the CRM system. Identify opportunities for health promotion and illness prevention. Maintain knowledge of community resources and payor requirements for the target population. Meet all performance standards established for the position. Perform other duties as assigned by management.
