Job Description Summary The Clinical Pharmacist is a licensed professional who is responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the policies and procedures of Option Care, pharmacy practice professional standards, and applicable regulatory entities. This entry-level position introduces the clinical pharmacy practice in the alternate setting. The staff Clinical Pharmacist coordinates patient services with the Nursing Department, monitors patient progress toward goals, and educates patients, caregivers, staff and other professionals regarding appropriate medication use. Job Description Provides medication use education to staff, patients, caregivers, and other professionals; coordinates patient services; monitors patient progress; develops patient-specific pharmacy care plans; ensures continuity of pharmaceutical care; manages medication administration equipment; responds to drug information requests; maintains confidentiality; complies with all accreditation, legal, regulatory, and safety requirements. Job Responsibilities Determines suitability of individual patients for home care; collects and organizes necessary patient‑specific information. Designs, recommends, monitors, and evaluates patient‑specific pharmacy care plans; recommends modifications to medication therapy or monitoring parameters as appropriate. Modifies pharmacy care plans based on monitoring data and other patient‑specific information; competently applies pharmacokinetic principles and evaluates laboratory values. Provides medication‑use education to staff and patients using effective counseling techniques. Ensures continuity of pharmaceutical care to and from the home and other settings; communicates pertinent information systematically. Prepares and dispenses medications following Option Care policies, safety, and quality assurance practices; calibrates equipment, prepares labels, and ensures proper storage. Manages medication administration equipment; programs, maintains, and advises other staff; documents and reports equipment incidents. Responds to drug information requests with literature‑based responses; attends CEU opportunities to maintain competency. Manages investigational drug products according to policies and procedures; utilizes support personnel and prioritizes workload. Complies with accreditation, legal, regulatory, and safety requirements in all activities. Maintains confidentiality of patient and proprietary information; observes legal and ethical confidentiality guidelines. Basic Education and/or Experience Requirements Graduate of an accredited School of Pharmacy. Current, active Pharmacy license granted by the State Board of Pharmacy. One of the following: at least one year of experience in sterile IV admixture procedures; knowledge of non‑sterile compounding, immunizations and medication therapy management; completion of an ASHP accredited Pharmacy Residency Program; completion of an Advanced Pharmacy Practice Experience associated with an accredited school; or equivalent experience approved by the VP of Pharmacy or SVP of Clinical Services. Basic Qualifications Experience providing customer service to internal and external customers, meeting quality standards and evaluating customer satisfaction. Basic proficiency in Microsoft Word for document editing and formatting. Basic proficiency in Microsoft Excel for worksheet management and data handling. Basic proficiency in Microsoft PowerPoint for presentation creation and editing. Travel Requirements Willing to travel up to 5% of the time for business purposes within the state. Preferred Qualifications & Interests Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience. Experience in home care, hospital, or other clean room settings. Management Requirements At least one year of experience in indirect management of team members, including assisting in development, training, and assignment of work/projects to other team members. Salary and Benefits Salary determined by education, experience, knowledge, skills, and abilities. Pay range: $117,548.49 – $195,936.65.
