Physical Therapist

Physical Therapist - Skilled Nursing Facility. Monticello, MN. We are seeking a Physical Therapist for a full-time role at a skilled nursing facility in Monticello, MN. The clinician will evaluate, develop, and implement individualized treatment plans to improve patient mobility, functional independence, and quality of life. The position involves working with post-acute and long-term care populations and coordinating care with interdisciplinary team members.
