In this meaningful role, you'll play a vital part in guiding young people through critical transitions, connecting them with the resources and support they need to thrive. Responsibilities Develop, implement, and monitor individualized case plans for youth residents Coordinate services and referrals to community resources, including mental health, education, and employment support Conduct regular check-ins and assessments to track client progress and adjust care plans as needed Maintain accurate and up-to-date case documentation and client records in compliance with program standards Collaborate with internal staff, external agencies, and family members to ensure holistic support for each youth Facilitate life skills workshops and group sessions to promote independence and self-sufficiency Advocate for youth needs within the organization and with outside service providers Requirements Bachelor's degree in Social Work, Psychology, Human Services, or a related field preferred Prior experience in case management, youth services, or social services strongly preferred Knowledge of trauma-informed care and youth development principles Strong organizational skills with the ability to manage multiple cases simultaneously Excellent written and verbal communication skills Ability to work collaboratively with a multidisciplinary team and community partners Valid driver's license and reliable transportation required Empathetic, patient, and dedicated to empowering at-risk youth About Us Transitional Residence for Youth is a dedicated organization in Topeka, KS, committed to providing safe, supportive housing and comprehensive services for young people navigating life's most challenging transitions.
