Role Overview As a Case Manager, you’ll be a vital member of our patient-centered care team, ensuring seamless coordination and advocacy for patients and their families. Your role is to identify, assess, and plan care strategies that promote safety, quality, and cost-effective outcomes. You’ll facilitate transitions to the next level of care, manage critical diagnoses, and optimize health outcomes across the care continuum, making a direct impact on patient wellbeing. What You Will Do Collaborate with interdisciplinary teams to assess patient needs and develop individualized care plans. Coordinate discharge planning to ensure safe transitions to the next level of care. Advocate for patients and families by connecting them with community resources and support services. Evaluate care outcomes and adjust strategies to optimize health and wellness. Promote patient safety, quality of care, and cost-effective solutions across the care continuum. Qualifications Required Qualifications Bachelor's degree in nursing or Master's degree in social work. Minimum 3 years of clinical experience. Valid, unrestricted State of Michigan license as a Registered Nurse (RN) or Licensed Master Social Worker (LMSW). Excellent verbal communication, written documentation, and interpersonal skills. Strong problem-solving, analytical, and decision-making abilities. Proficiency with computer systems and healthcare software applications. Preferred Qualifications Certification in Case Management (CCM) or Accredited Case Manager (ACM). Experience in discharge planning, home health care, rehabilitative medicine, or managed care.
