Maintain oversight of assigned clients and monitor their ongoing status, functioning, safety, and well-being.
Evaluate clients’ capacity to manage their own affairs and identify potential risks, including financial exploitation, neglect, and other safety concerns.
Screen clients for program eligibility, conduct triage for emergency interventions, and assess immediate and ongoing service needs.

Assist with implementing court-ordered protective actions and coordinating emergency interventions.
Work collaboratively with supervisors and external service providers to address crisis situations and protect client well-being.
Be available to work outside the regular schedule, including evenings, weekends, and holidays, when required to respond to emergencies or meet program needs.
