The Life Skills Clinician - School-Based (Warren Township) provides therapeutic and skill-building services to support youth in gaining practical, social, and emotional abilities to navigate daily life successfully. This role delivers services in multiple settings, but primarily in schools. The Life Skills Clinician- School-Based focuses on strengthening daily living and school related skill sets while collaborating with the interdisciplinary care team to support the student and their family. There is potential that providers may have to float between sites and between school districts to cover for other providers when appropriate. Essential Functions & Key Responsibilities The essential duties listed below represent the primary responsibilities of this role. Other related duties may be assigned.- Build therapeutic rapport with patients and families from diverse cultural, linguistic, and socioeconomic backgrounds
- Engage patients who are difficult to reach using motivational interviewing and trauma-informed approaches
- Partner closely with licensed behavioral health, school, and medical providers to review care plans and identify patient-specific life skills goals
- Provide individualized, family, and group-based life skills training tailored to patient/family needs and treatment goals
- Support patients in developing emotion regulation, organization, social, self-care, conflict-resolution, and problem-solving skills, as well as promote positive health behaviors
- Prepare patients in transition planning for life after school, including vocational skills, community participation, and independent living
- Collaborate with school staff, families, and natural supports to ensure skills are reinforced consistently across all environments
- Assist parents/guardians in navigating healthcare systems, public transportation, government agencies, and community organizations
- Conduct home visits to assess the patient's living environment and provide skills and parent training in a real-world context
- Accompany patients and families to appointments or community activities as clinically indicated to build independence
- Identify community resources and establish collaborative relationships with local organizations to support patient needs
- Participate in school and care team meetings, case consultations, and supervision to ensure integrated, patient-centered care
- Communicate regularly with licensed behavioral health providers, school staff, primary care providers, and social services regarding patient progress, barriers, and changes in status
- Assist in coordinating referrals to community resources, social services, housing, employment, and other supportive services
- Maintain accurate and timely clinical documentation in the Electronic Health Record (EHR)
- Apply knowledge of protective service mandates for both adults and children
- Utilize risk screens, de-escalation, and crisis intervention techniques when needed
- Other duties may be assigned
- Working knowledge of child and adolescent development, social determinants of health and the challenges facing underserved and low-income populations
- Familiarity with local community resources and social services systems
- Excellent interpersonal, written, and verbal communication skills
- Ability to pass a background check and drug screen in accordance with FQHC and regulatory requirements
- Valid driver's license required
- CPI (Non-Violent Crisis Intervention) certification preferred
- Bilingual or multilingual fluency preferred
- Bachelor's Degree in Social Work, Psychology, Human Services, or related field required or equivalent experience
- One year experience working within a public school district preferred
- Must be credentialed according to the requirements of the evidence-based model(s) used
- To perform this job successfully, an individual should have moderate computer skills, including Microsoft Office (specifically Word and Excel) and other applications.
